Quick answer
Save the template below as .github/pull_request_template.md and merge it to the default branch; GitHub then pre-fills every new PR body with it. For several templates, use a .github/PULL_REQUEST_TEMPLATE/ folder and the template query parameter (GitHub Docs, checked 23 October 2026).
The template
## What and why
<!-- One or two sentences. Link the issue: Fixes #123 -->
## How to test
1.
2.
## Screenshots or recording
<!-- Before / after for any UI change. A 30-second recording beats five screenshots. -->
## QA checklist
- [ ] Tested the happy path and at least one error path
- [ ] Tested on the smallest supported screen (or not UI)
- [ ] Tests added or updated (or explained why not)
- [ ] No secrets, personal data or debug logging added
- [ ] Feature flag / config change documented (or none)
- [ ] Migration is reversible or rollback plan written (or no migration)
- [ ] Release note line written below (or internal only)
## Release note
<!-- One line in user language, e.g. "CSV exports now include all rows for large projects." -->Why each section is there
- What and why with "Fixes #123" links the issue (and closes it on merge on GitHub).
- How to test gives the reviewer steps, not just code to read.
- Screenshots or recording: a 30-second before/after recording shows a UI change faster than five screenshots.
- QA checklist catches the things code review misses: error paths, small screens, secrets, migrations.
- Release note written by the person who knows the change best.
Other hosts: GitLab uses .gitlab/merge_request_templates/; Azure DevOps and Bitbucket have their own settings for PR descriptions. The Markdown above works in all of them.
Record the video part
Record the before and after of a UI change and drag the file into the PR description. Bobbin is a free Chrome extension that records your screen, a window or a tab with your microphone and an optional webcam bubble. No account, no watermark, no time limit, and nothing is uploaded: the file stays on your computer.
- Click the Bobbin icon in Chrome's toolbar and choose Record screen, window or tab.
- Pick your microphone and turn on the camera if you want the webcam bubble.
- Press Start recording, choose what to share in Chrome's picker, and wait for the 3-second countdown.
- Pause with Alt+Shift+P, stop with Alt+Shift+S, then press Download to save the file.
To keep other tabs and notifications out of it, share just the Chrome tab (the webcam bubble appears only in screen and window recordings). Bobbin saves MP4 where your browser supports it (Chrome 126 or newer), WebM otherwise, and tells you which it used.
File size: our 60-minute 1080p test came to about 246 MB (roughly 4 MB a minute) on a simple synthetic screen; busy real screens can be larger, so a 1–2 minute clip is usually small enough to attach.
