Quick answer
Record seven things: product and version, date and owners, scope link, checklists completed, test results (passed / failed / not run), open issues shipping with their severity and who accepted them, the rollback plan, and the decision with the decider's name and time.
The template
Release sign-off: [PRODUCT] [VERSION]

Scope: [LINK TO RELEASE NOTES]
Checklists completed: [R1 / R2 / R3 / R4 / R8 / R9] (links)
Test results: [X] passed, [Y] failed, [Z] not run (reason: [REASON])
Open issues shipping with this release: [LIST WITH SEVERITY], accepted by [NAME]
Rollback plan: [LINK OR COMMAND]
Decision: GO / NO-GO Decided by: [NAME] at [TIME + TIMEZONE]Using it for UAT
- Set Scope to the UAT scope or acceptance criteria.
- Under Test results, count the UAT scenarios the business users ran.
- The decider is the business owner who accepts the release, not the delivery team.
For an Excel or Word version, paste the block into a two-column table (field, value).
Record the video part
For UAT, a 2-minute walkthrough recording of the accepted flow is a useful attachment to the sign-off. Bobbin is a free Chrome extension that records your screen, a window or a tab with your microphone and an optional webcam bubble. No account, no watermark, no time limit, and nothing is uploaded: the file stays on your computer.
- Click the Bobbin icon in Chrome's toolbar and choose Record screen, window or tab.
- Pick your microphone and turn on the camera if you want the webcam bubble.
- Press Start recording, choose what to share in Chrome's picker, and wait for the 3-second countdown.
- Pause with Alt+Shift+P, stop with Alt+Shift+S, then press Download to save the file.
Share a window or the entire screen so your webcam bubble is in the video (it doesn't appear in single-tab recordings). Bobbin saves MP4 where your browser supports it (Chrome 126 or newer), WebM otherwise, and tells you which it used.
File size: our 60-minute 1080p test came to about 246 MB (roughly 4 MB a minute) on a simple synthetic screen; busy real screens can be larger, so a 1–2 minute clip is usually small enough to attach.
From The Bug Report & QA Kit
The kit has the R1–R9 checklists this sign-off links to. The full kit: 10 bug templates for GitHub, Jira, Linear and Markdown, 9 release checklists (126 checks), the severity guide, 8 bug-video scripts and trackers.
